Description
Posterior neck and upper trunk are presented with the intermediate layer of deep fascia rendered as a distinct sheet separating the superficial trapezius from deeper scapular and paraspinal compartments. The trapezius spans the midline from the external occipital protuberance and nuchal ligament inferiorly toward the acromion and scapular spine, while the deltoid caps the lateral shoulder; deep to the trapezius, the rhomboid major and minor and the upper latissimus dorsi are suggested along the medial border and inferior angle region of the scapula. Medially, the erector spinae mass sits in longitudinal columns adjacent to the spinous processes, contained by fascial partitions continuous with the thoracolumbar fascia. Clean planes matter here. Fascial anatomy in this region often gets reduced to “just connective tissue,” yet the intermediate layer is the surgical plane you meet when elevating trapezius for posterior cervical exposure, scapular fixation approaches, or harvesting a trapezius myocutaneous flap. Those septa also help explain compartmental spread of infection or hematoma between the superficial investing fascia and deeper paraspinal fascia, and why injections or regional blocks can track along aponeurotic planes. The retinacular and aponeurotic character of the sheet is easiest to appreciate from the posterior shoulder girdle, where fascial tension lines parallel muscle fiber direction and scapular movement. Use this plate in gross anatomy teaching when you want students to stop “peeling muscles” and start identifying real dissection planes, and in surgical education materials covering posterior neck incisions, scapular approaches, and flap elevation over the upper back. It also fits well in anatomy atlases and physiotherapy texts discussing myofascial continuity across the nuchal region and thoracolumbar fascia.
