Deep Cervical Fascia

Description

Posterior neck anatomy is presented with emphasis on the deep cervical fascia and its compartmental organization beneath the integument. The investing layer envelops the trapezius bilaterally and spans laterally toward the sternocleidomastoid, while deeper laminae partition the posterior cervical region from the visceral and vascular spaces. The carotid sheath is distinguished as a separate fascial tube positioned anterolateral to the cervical vertebral column and medial to the sternocleidomastoid, maintaining a clear boundary from adjacent neck compartments. Understanding these fascial separations matters when you teach or plan approaches to the neck because the deep cervical fascia governs how infection, hematoma, and emphysema track between spaces, and it also defines the surgical “feel” of safe dissection. The carotid sheath is a frequent landmark in carotid endarterectomy and anterior cervical spine exposures, where identifying its relationship to the pretracheal and prevertebral fascia helps avoid unintended entry into the vascular compartment. Posteriorly, appreciating how the investing fascia invests trapezius and blends toward the nuchal region supports interpretation of deep neck collections that can present with posterior neck swelling rather than the classic anterior bulge.
