Description
Posterior neck and upper back anatomy is presented with the superficial tissues removed to expose the deep cervical fascia and its laminar organization. Across the nuchal region, the investing layer of deep cervical fascia lies superficial to the splenius capitis and semispinalis capitis and continues inferolaterally to envelop the trapezius, which spans from the external occipital protuberance and superior nuchal line down toward the spine of the scapula. Deep to this, the prevertebral layer forms a fascial sleeve around the prevertebral and paraspinal musculature, fixing to the base of the skull superiorly and blending inferiorly with the endothoracic fascia. Laterally, the carotid sheath is typically oriented anterolateral to the cervical vertebral column, even when only its posterior relationships are emphasized. Seeing these fascial planes matters because they define how the neck compartmentalizes infection, blood, and air. The prevertebral fascia creates the floor of the retropharyngeal and danger spaces, a pathway for odontogenic or pharyngeal sepsis to track into the posterior mediastinum, while the investing fascia and trapezial envelope explain why posterior cervical abscesses can localize or spread along muscle borders. This is surgical anatomy. Posterior approaches to the cervical spine and posterior triangle dissections rely on staying oriented to the investing layer and the deeper prevertebral sleeve to avoid drifting into the wrong plane.
