Description
Seen in lateral profile, the investing layer of the deep cervical fascia forms a continuous fibrous collar around the neck, splitting to enclose the sternocleidomastoid anteriorly and the trapezius posteriorly. Superiorly it blends with the deep fascia at the inferior border of the mandible and parotid region, while inferiorly it anchors to the clavicle and manubrium, bridging across the posterior triangle. Superficial facial musculature, the temporalis over the temporal fossa, and the occipital belly of occipitofrontalis sit superficial to the cranial aponeurosis, with the auricle marking the lateral boundary between scalp and upper cervical soft tissues. Fascial planes matter in the neck because they guide both surgical dissection and the spread of infection. The investing layer defines the roof of the anterior and posterior triangles and provides the sheath-like investment that is encountered early in cervical approaches to lymph node levels II to V and during exposure for carotid endarterectomy, even though the carotid sheath lies deeper. Tightness or edema in this layer can also help explain why superficial neck infections may track along the posterior border of the sternocleidomastoid toward the clavicle rather than directly medially.
