Description
Seen from a right lateral perspective, the crural deep fascia (fascia cruris) forms a dense investing sheath around the leg, enclosing the gastrocnemius and soleus posteriorly while continuing anteriorly over the lateral compartment. Septa extending medially and laterally from this fascia tether to the tibial and fibular borders, partitioning the anterior, lateral, and posterior compartments in relation to the tibia (medial) and fibula (lateral). Proximally the fascia blends near the knee with the fascia lata and crural expansions around the lateral femoral condyle, and distally it thickens toward the ankle as it contributes to the superior extensor retinaculum anteriorly and the superior fibular (peroneal) retinaculum laterally. Bony landmarks of the tibia, fibula, and ankle-foot skeleton provide the underlying framework. Compartment anatomy is the point. A lateral view makes the fibular side easy to teach, including the osteofascial boundaries that trap swelling in acute compartment syndrome and threaten the deep fibular nerve and anterior tibial vessels in the anterior compartment. Surgical fasciotomy incisions are planned with this fascial architecture in mind, and the relationship of the retinacula to the lateral malleolus explains why peroneal tendon subluxation follows retinacular injury and why extensor tendons can be constricted in anterior ankle tenosynovitis. Tight fascia changes everything. Use this illustration for gross anatomy and musculoskeletal courses when introducing leg compartments, retinacula, and the continuity of deep fascia across the knee and ankle, and for operative textbooks discussing leg fasciotomy, lateral ankle approaches, or peroneal tendon stabilization.
