Description
Running from the tibial tuberosity to the anterior ankle, the crural deep fascia (fascia cruris) forms a dense sheath over the anterior compartment of the lower leg, lying superficial to tibialis anterior, extensor hallucis longus, and extensor digitorum longus and blending medially with the subcutaneous border of the tibia. Laterally it invests the fibular (peroneal) region and sends intermuscular septa posteriorly toward the fibula, separating the anterior compartment from the lateral and posterior compartments. Distally, the fascia thickens into the superior and inferior extensor retinacula, spanning the anterior aspect of the ankle to hold the extensor tendons close to the talocrural joint. Bony landmarks such as the patella, tibial crest, fibula, and malleoli orient the relationships. This anterior emphasis is the practical way to teach how deep fascia creates closed osteofascial spaces in the leg, the anatomy that makes acute compartment syndrome a limb-threatening diagnosis after tibial fractures, crush injury, or reperfusion. The extensor retinacula also matter in tendon pathology and postoperative scarring, where tethering can limit dorsiflexion and alter toe extension mechanics. A tight compartment has consequences.
