Description
Posterior lower limbs are rendered with the deep transverse fascia (transverse intermuscular septum of the leg) emphasized as a fibrous connective sheath spanning between the tibia medially and fibula laterally. Superficial to this septum lie the gastrocnemius heads and soleus tapering inferiorly into the calcaneal (Achilles) tendon, while deep to it sit the deep posterior compartment muscles, including tibialis posterior, flexor digitorum longus, and flexor hallucis longus, arranged close to the posterior surfaces of the tibia and fibula. Proximally, the hamstrings (biceps femoris laterally, semitendinosus and semimembranosus medially) frame the popliteal region superior to the calf, and distally the fascia continues toward the medial malleolar region where the deep tendons course toward the foot. Compartment anatomy is the point. The deep transverse fascia defines the boundary between the superficial and deep posterior compartments, a relationship that drives both symptom patterns and management in acute compartment syndrome following tibial fractures, crush injury, or reperfusion after vascular occlusion. It also underpins surgical decompression planning: a posterior fasciotomy must address the deep compartment separately, and the septum can prevent adequate pressure relief if left intact.
