Description
Posterior thoracic anatomy is rendered with the superficial back muscles intact, including trapezius spanning from the nuchal region to the thoracic spinous processes and laterally to the acromion and scapular spine, with deltoid capping the glenohumeral region. Inferiorly, latissimus dorsi sweeps superolaterally from the thoracolumbar fascia toward the humerus, while the erector spinae columns run longitudinally on either side of the vertebral midline. Along the posterolateral chest wall, the serrati fascia is emphasized as a distinct fascial sheath overlying the serratus anterior region, positioned deep to the scapular musculature and superficial to the rib cage, tracking laterally toward the midaxillary line. Serrati fascia matters when you need to explain why the serratus anterior glides smoothly over the ribs and why fluid, blood, or infection can dissect along fascial planes of the lateral thoracic wall. This posterior perspective supports teaching scapulothoracic mechanics: the serratus anterior lies anterior to the scapula and ribs, yet its fascial envelope relates closely to the posterior axillary fold formed by latissimus dorsi, a relationship that helps orient clinicians during posterolateral thoracotomy, chest tube placement, or regional anesthesia planning around the long thoracic nerve (C5 to C7), a structure whose injury produces medial scapular winging.
