Description
Over the posterior shoulder, the supraspinous fascia forms a dense aponeurotic roof over the supraspinous fossa, spanning from the spine of the scapula inferiorly toward the superior border of the scapula and blending medially with periosteum near the medial border. Deep to this fascial sheath sits the supraspinatus muscle, its fibers running laterally beneath the acromion toward the greater tubercle of the humerus, while the scapular spine marks the inferior boundary of the compartment and separates it from the infraspinous fossa. Bony landmarks such as the acromion, lateral angle of the scapula, and proximal humerus provide orientation for the cuff interval and the subacromial space. Small, firm anatomy. Treating this fascia as a retinaculum helps explain why the supraspinatus belly and tendon glide in a confined passage before reaching the subacromial bursa and the superior facet of the greater tubercle. In rotator cuff disease, that tight relationship between the supraspinous compartment, the acromion, and the supraspinatus tendon is where impingement signs, bursal-sided tearing, and postoperative adhesions tend to concentrate, and it is also the plane you respect when elevating deltoid and cuff tissues during an open or mini-open cuff repair. The view supports teaching of compartment boundaries and why decompression targets the roof over the tendon rather than the muscle belly itself.
