Description
Over the posterior shoulder girdle, the supraspinous fascia is rendered as a dense aponeurotic sheet spanning the supraspinous fossa of the scapula, investing the supraspinatus muscle as it courses laterally toward the superior facet of the greater tubercle of the humerus. Superiorly it relates to the spine of the scapula and the overlying trapezius, while deep to it the supraspinatus belly occupies the fossa and transitions into tendon beneath the acromion. Laterally, the fascial envelope blends with the rotator cuff fascia near the greater tubercle and approaches the subacromial space, where the tendon, bursa, and coracoacromial arch form a tight functional corridor. Bone landmarks, including the scapular spine, acromion, glenoid region, and humeral head, anchor the spatial anatomy. Tough tissue. Tight space. That fascial roof matters because it sets the constraint conditions for supraspinatus excursion and tendon loading during abduction, particularly as the tendon passes under the acromion and coracoacromial ligament. In rotator cuff disease, bursal-sided supraspinatus tendinopathy and partial-thickness tears often localize to this corridor, and surgeons working arthroscopically in the subacromial space must constantly orient to the scapular spine and acromial undersurface while protecting cuff tissue. This view also clarifies how an investing fascia can behave like a retinaculum, resisting muscle belly bulging and directing force transmission into the tendon. Use it in gross anatomy and kinesiology teaching to differentiate supraspinous fascia from the overlying trapezial fascia and to explain why supraspinatus is clinically overrepresented in impingement-type shoulder pain. It also fits rotator cuff chapters in orthopedic texts, arthroscopy atlases, and patient-education materials discussing subacromial decompression or cuff repair planning.
