Description
Layered over the posterior thoracic wall, the levatores costarum appear as short, obliquely oriented slips arising from the transverse processes of the lower cervical and upper thoracic vertebrae and inserting onto the rib immediately inferior, near the tubercle and angle. Each fascicle runs inferolaterally from the vertebral column toward the rib cage, positioned deep to the erector spinae mass and medial to the posterior scapular musculature. Portions of trapezius, rhomboid major and minor, levator scapulae, posterior deltoid, and latissimus dorsi frame the region, with the scapulae lying lateral to the spinous processes and the upper ribs curving anterolaterally beneath the muscle layers. Fiber direction is the story here. For teaching thoracic mechanics, this posterior perspective clarifies how the levatores costarum bridge vertebrae to ribs and how their line of pull can assist rib elevation during inspiration, while also acting as segmental stabilizers of the thoracic spine. Clinically, these muscles sit in the same deep paraspinal corridor approached during thoracic paravertebral block and can be mistaken for adjacent intercostal or multifidus fibers when mapping pain generators in costovertebral dysfunction. Their proximity to the rib angle makes them a plausible source of focal posterior rib pain and a frequent site of myofascial trigger points that refer along the intercostal spaces. Anatomy faculty will find it suited to thoracic wall and back dissection guides, rib and vertebral landmark teaching, and respiratory biomechanics modules in physiotherapy and osteopathic curricula. It also reads well in atlases covering posterior thoracic approaches, regional anesthesia, and differential diagnosis of posterior thoracic pain.
