Description

Centered in the anterior thorax, the trachea descends in the midline from the lower neck to the carina, where it bifurcates into the right and left main bronchi. Each bronchus enters the lung at the hilum, with the right lung showing superior, middle, and inferior lobes separated by the horizontal and oblique fissures, and the left lung showing superior and inferior lobes divided by the oblique fissure. Lung apices project superior to the clavicles, while the bases rest inferiorly on the diaphragm and fill the costal recesses laterally. Upper airway context is included, with the nasal passage continuing inferiorly toward the laryngeal inlet and into the trachea. Anterior orientation makes the tracheobronchial tree easy to teach in relation to surface landmarks used in auscultation and airway management. The carina is the key reference point when confirming endotracheal tube depth, and this view also supports discussion of why aspirated material and misplaced tubes more often enter the wider, more vertical right main bronchus. Lobar anatomy remains clear enough to map focal findings, for example right middle lobe atelectasis that can be subtle on a frontal exam.
