Description
Centered in the right upper quadrant, the gallbladder sits on the visceral (inferior) surface of the liver, its fundus projecting anteriorly toward the abdominal wall while the body lies nestled in the gallbladder fossa. From the neck, the cystic duct courses medially and posteriorly to join the common hepatic duct and form the common bile duct, which then descends inferiorly toward the second part of the duodenum. Intrahepatic bile ducts converge at the porta hepatis superior to the extrahepatic biliary tree, and the distal common bile duct approaches the pancreatic head where it relates closely to the main pancreatic duct. Surface anatomy is suggested by the anterior overlay on an obese adult male torso. An anterior close-up like this helps when teaching how biliary anatomy maps to pain patterns, palpation landmarks, and procedural access, including the region of Murphy sign tenderness near the right costal margin in cholecystitis. Obesity changes the depth and sonographic window for gallbladder ultrasound and can complicate laparoscopic cholecystectomy by obscuring Calot triangle relationships between the cystic duct, common hepatic duct, and cystic artery. Ductal anatomy matters. A small variant or a low cystic duct insertion is a setup for bile duct injury.
