Description
Positioned in the right upper quadrant, the gallbladder (vesica biliaris) sits on the visceral (inferior) surface of the liver, with its fundus projecting anteriorly and inferiorly toward the right costal margin. From an anterior perspective, only the dome-like fundus is expected to be visible, peeking out from beneath the inferior hepatic border, while the body and neck remain tucked posteriorly in the gallbladder fossa. Surrounding surface landmarks include the right hypochondriac region and the upper abdominal wall. The liver covers most of it. Clinically, that small anterior “tip” of gallbladder fundus matters because it corresponds to the region palpated in acute cholecystitis and helps explain where patients localize pain during a positive Murphy sign under the right costal margin. In an obese male habitus, increased subcutaneous tissue and altered abdominal wall contour can blunt physical exam findings and complicate bedside assessment, which is why correlating surface anatomy with the true subhepatic position of the gallbladder remains a core teaching point. Surgeons also use the fundus as an initial landmark during laparoscopic cholecystectomy when working from an anterior approach before dissecting toward Calot triangle. Ideal uses include undergraduate anatomy and hepatobiliary modules, clinical skills curricula covering right upper quadrant examination, and patient-facing education that explains biliary colic and cholecystitis with relatable external landmarks.
