Description

Centered on the anterior abdominal wall, the large intestine is traced from the right iliac fossa where the cecum sits inferior and lateral, then superiorly along the ascending colon toward the hepatic flexure beneath the right costal margin. The transverse colon spans right to left across the upper abdomen, lying inferior to the stomach region and anterior to much of the small-bowel mass, before turning at the splenic flexure into the descending colon along the left flank. Distally, the sigmoid colon curves inferomedially toward the midline en route to the rectum. Haustra and the longitudinal teniae coli define the segmented contour, while epiploic appendages and prominent mesenteric and omental fat reflect an obese male habitus. For teaching and clinical communication, an anterior surface projection like this helps translate intraperitoneal anatomy to what you palpate and what you plan around. The expanded right colon and its relationship to the right lower quadrant is a practical anchor when discussing appendiceal pathology, cecal volvulus, or right-sided diverticular disease, all of which can present with nonclassic pain patterns in patients with increased abdominal adiposity. Increased visceral fat also affects colonoscopy torque and loop formation, and it changes operative exposure during a laparoscopic right hemicolectomy where the hepatic flexure mobilization occurs close to the liver and omentum. Good landmarks, not guesswork.
