Description
Beneath the dermal layer of the male back, the multifidus is exposed as a deep paraspinal muscle mass running immediately posterior to the vertebral arches and medial to the erector spinae group. Short, obliquely oriented fascicles span from mammillary processes in the lumbar region and transverse processes more superiorly to the spinous processes of vertebrae two to four segments above, creating a stepped pattern along the midline. Lumbar fibers appear thicker and more densely layered than the cervical portion, where the muscle narrows and blends with other deep extensors near the posterior elements. Red muscle bellies contrast against pale vertebrae and adjacent ribs. Lumbar multifidus bulk and segmental fiber direction are not just academic details, they are the substrate for intersegmental control of the lumbar spine. Atrophy and fatty infiltration of multifidus commonly follow acute low back pain and persist after lumbar discectomy or laminectomy, a pattern often discussed alongside MRI findings and motor control retraining. This close view also supports procedural teaching: medial branch blocks and radiofrequency ablation target the dorsal rami near the junction of the superior articular and transverse processes, a neighborhood where multifidus innervation and attachments explain post-procedure soreness and denervation-related muscle changes.
