From the lateral perspective, the muscle mass fills the groove between the spinous processes medially and the transverse processes and rib angles laterally, sitting deep to the erector spinae and hugging the posterior elements of the vertebral column. The adjacent ribcage arcs anteriorly from the thoracic spine, and the pelvis anchors the caudal origin near the posterior superior iliac spine and sacroiliac region. Segmental anatomy reads clearly. Multifidus matters because its segment-by-segment attachment pattern makes it a primary stabilizer of intervertebral motion rather than a gross extensor, and that distinction is central when explaining mechanical low back pain.
