Description
Arising from the transverse processes and inserting on the laminae and spinous processes of the adjacent lumbar vertebrae, the rotatores lumborum are shown as short, deep slips within the transversospinalis group, lying medial to the more obliquely oriented multifidus. From a lateral aspect of the male trunk, the thoracic cage frames the superior field while the lumbar vertebral column and posterior pelvis anchor the inferior field, with the rotatores tucked posterior to the vertebral bodies and immediately deep to the vertebral arches. Segmental layering is the point: transverse process laterally, lamina posteriorly, spinous process at the midline. These tiny muscles matter because they sit where mechanical back pain is often generated and where clinicians struggle to correlate imaging with palpable landmarks. Their intimate relationship to the zygapophysial (facet) joints and interspinous region makes them relevant when teaching lumbar segmental control, and when discussing why medial branch blocks or radiofrequency ablation target nociception from the posterior elements rather than the rotatores themselves. Small. Easy to overlook.
