Description
Running along the posterior vertebral column, the rotatores lumborum occupy the deepest stratum of the transversospinalis group, tucked in the lumbar gutter adjacent to the laminae. Short muscular slips span from the transverse processes to the junction of lamina and spinous process of the vertebra above, lying deep to multifidus and semispinalis, and immediately posterior to the vertebral arches. A posterior view also situates these fibers relative to the ribs superiorly and the pelvis inferiorly, with the lumbar segments framed between the lower thoracic cage and the iliac crests. Deep and easy to miss. Small as they are, rotatores matter when you are teaching segmental stability and the layered organization of intrinsic back musculature, because they mark the transition from broad erector spinae masses to the fine, proprioceptive system that couples adjacent motion segments. Their attachments near the facet joints make them relevant to lumbar zygapophysial pain patterns and to surgical planes used in posterior approaches where preservation of the multifidus and deeper transversospinalis can influence postoperative function. Palpation will not isolate them, so a clear schematic view helps clinicians and students conceptualize what lies immediately on the posterior elements once the superficial muscles are reflected.
