Description
Running along the posterior vertebral column, the rotatores muscles appear as short, obliquely oriented slips spanning from the transverse process to the lamina and spinous process of the vertebra immediately superior, most developed in the thoracic region. Deep to the multifidus, they sit medial to the longissimus thoracis mass and adjacent to the interspinales, which bridge between neighboring spinous processes in the midline. Ribs articulate laterally at the costovertebral and costotransverse joints, framing the thoracic cage while the lumbar vertebrae and sacrum form the inferior base of the field. Small muscles. Tight real estate. Posterior deep back anatomy matters because the rotatores and multifidus anchor the segmental control of the thoracolumbar spine, and their innervation by the medial branches of the dorsal rami makes them key landmarks in both pain generation and intervention.
