Posteriorly, the deep rotatores lumborum appear as short, obliquely oriented slips running from the transverse processes to the laminae and spinous processes of the lumbar vertebrae, one to two segments superior. Nestled medial to the longissimus thoracis and iliocostalis lumborum and deep to multifidus, these myofibers sit immediately lateral to the spinous processes along the lumbar midline. The thoracolumbar fascia forms the superficial envelope over the paraspinal compartment, while the sacrum and posterior iliac crests define the inferior bony frame of the low back. Small and easily overlooked. Appreciating the rotatores in a full body posterior view helps clarify why “paraspinal” is not a single layer, a point that matters when correlating lumbar pain patterns with targeted examination or intervention. Their segmental orientation, spanning adjacent lumbar levels, is commonly discussed in relation to fine intersegmental control and proprioception, and it becomes clinically relevant when placing deep medial branch blocks or radiofrequency ablation probes along the course of the dorsal rami that innervate the multifidus-rotatores complex. This angle also supports teaching the posterior surgical corridor to the lumbar spine, where splitting or stripping posterior musculature off the lamina can disrupt the deepest transversospinal fibers.
