Description
Posterior lumbar anatomy is centered on the rotatores lumborum, the shortest slips of the transversospinales group, spanning from the transverse process of a lower vertebra to the lamina and spinous process of the vertebra immediately superior. These delicate fascicles sit deep to semispinalis and multifidus, hugging the posterior elements of the lumbar vertebrae and aligning close to the zygapophysial (facet) joints. Surrounding bony landmarks visible from this rear perspective include the spinous processes in the midline, paired transverse processes laterally, the sacrum inferiorly, and the posterior iliac crests framing the lumbosacral region. Scapulae, ribs, and the occiput provide broader orientation in the upper back. Because the lumbar rotatores are so short, this view is most useful for teaching depth relationships within the intrinsic back muscles rather than for gross surface anatomy. Their proximity to the facet joints and intervertebral motion segments makes them relevant when discussing segmental stabilization, postoperative pain after posterior lumbar approaches, and the anatomic basis for targeted interventions such as medial branch blocks and radiofrequency ablation performed for facet-mediated low back pain.
