Description

Running along the posterolateral aspect of the thoracolumbar vertebral column, the rotatores are rendered as short, obliquely oriented slips arising from the transverse processes and coursing superomedially to the lamina and spinous process of the vertebra one or two levels superior. Deep to semispinalis and multifidus, these tendinous fibers sit immediately posterior to the zygapophysial (facet) joints, with intervertebral discs interposed between adjacent vertebral bodies anteriorly. Spinal nerve roots exit through the intervertebral foramina, positioned anterolateral to the rotatores and adjacent to the pedicles. Rotatores matter because they are the deepest, most segmentally organized portion of the transversospinalis group, a relationship that is easy to lose when learners only see the more superficial erector spinae mass. Their short lever arm makes them less about gross extension and more about fine control, proprioception, and small-amplitude ipsilateral rotation, the exact mechanics implicated in facet-mediated pain patterns and in postoperative or postural motor control deficits.
