Posterior thoracolumbar back anatomy is rendered with the spinous processes of the thoracic and lumbar vertebrae in the midline, flanked by the paraspinal muscle columns. Superficial to deep, the erector spinae mass lies lateral to the spinous processes, while the transversospinal layer is opened to expose multifidus and the rotatores muscles closest to the laminae. Individual rotatores breves and longi can be inferred running superomedially from transverse process to the lamina and base of the spinous process of the vertebra above, crossing one segment (breves) or two segments (longi) and sitting medial to the bulk of longissimus and iliocostalis. Small. Segmental. Deep. Rotatores matter because they are the clinician’s map to the facet joints and the dorsal rami in the posterior approach to the spine. Their short oblique fibers overlie the zygapophysial joints and are commonly traversed during lumbar medial branch blocks and radiofrequency ablation for facet-mediated low back pain; knowing their segmental arrangement helps explain why pain generators can be localized to specific levels. This view also supports teaching of lumbar multifidus and rotatores atrophy patterns seen in chronic low back pain and after posterior surgical exposures. Use this illustration in gross anatomy and musculoskeletal anatomy courses when introducing the transversospinal group, and in spine surgery or pain medicine teaching materials that describe posterior midline approaches, laminotomy landmarks, and medial branch target levels.
