Description

Running in the deepest layer of the intrinsic back musculature, the rotatores appear as short, oblique fascicles coursing superomedially from the transverse process toward the lamina and spinous process of the vertebra above. Rotatores breves span a single motion segment, while rotatores longi cross two vertebrae, lying deep to multifidus and adjacent to the zygapophysial (facet) joints. Between successive vertebral bodies, the intervertebral discs form the anterior column, while spinal nerves exit laterally through the intervertebral foramina, immediately related to the pedicles and the posterior elements. Gray ligamentous bands along the posterior arch suggest the ligamenta flava and related posterior ligamentous complex that frames the vertebral canal. Orientation of the rotatores matters because their attachments place them in direct mechanical and proprioceptive relationship to the facet joints, a common generator of axial back pain. Their segmental pattern helps explain why small rotational and extension movements can reproduce symptoms in facet arthropathy and why targeted medial branch blocks address pain without treating the muscle itself. For surgical and interventional anatomy, the proximity of these deep fibers to the lamina and pars interarticularis also provides a practical reference when discussing posterior approaches and the soft-tissue corridor that must be traversed.
