Description
Running along the anterior compartment of the upper limb, the biceps brachii is shown with its long head positioned laterally and its short head medially, the two bellies converging into a single distal tendon as they approach the cubital fossa. Proximally, the long head tracks toward the intertubercular sulcus region of the humerus while the short head lies closer to the coracoid process area, creating the familiar superficial contour of the male arm. Distally, the tendon courses to the radial tuberosity and gives off the bicipital aponeurosis medially into the antebrachial fascia, with the brachialis deep to the biceps at the elbow and the brachioradialis forming the lateral border of the fossa. Forearm flexor and extensor muscle masses continue inferiorly toward the wrist, and intrinsic hand groups, including thenar and hypothenar eminences, are visible over the metacarpals and phalanges. Anterior biceps anatomy matters because the distal tendon and aponeurosis sit in the neighborhood of the brachial artery and median nerve within the cubital fossa, a relationship that becomes clinically concrete during venipuncture, elbow trauma assessment, and operative exposure. Distal biceps tendon rupture classically presents with loss of supination strength and a retracted muscle belly, and this view helps communicate why the radial tuberosity insertion and tendon course dictate the deformity pattern. A small sheet of fascia makes a big difference. The bicipital aponeurosis can mask complete tendon rupture on palpation and influences surgical approach and protection of the lateral antebrachial cutaneous nerve.
