Description
Anteriorly, the biceps brachii occupies the superficial compartment of the arm, spanning from the shoulder to the proximal forearm with its fusiform muscle belly tapering into a distal tendon. Proximally, the long head tendon courses within the intertubercular (bicipital) groove of the humerus while the short head arises from the coracoid process, the two heads merging in the mid arm anterior to the brachialis. Distally, the tendon passes anterior to the elbow joint to insert on the radial tuberosity, and the bicipital aponeurosis fans medially into the antebrachial fascia, superficial to the brachial artery and median nerve. Surface anatomy matters here. An anterior view of the male biceps brachii is a practical teaching angle because it clarifies how elbow flexion and forearm supination couple through the distal tendon’s attachment to the radius, not the ulna. That relationship underlies the clinical pattern of distal biceps tendon rupture, where patients often report a sudden pop with eccentric loading and demonstrate weakness in supination more than flexion, with proximal retraction producing the classic “Popeye” contour. The bicipital aponeurosis also explains why some partial tears preserve elbow contour while still compromising strength, and it provides a key landmark during venipuncture and anterior elbow approaches by draping over deeper neurovascular structures.
