Description
Medial anatomy of the male arm is presented from the shoulder through the hand, centering on the biceps brachii and its deep relationship to the brachialis along the anterior humerus. Proximally, the long head tendon tracks within the intertubercular (bicipital) groove toward the supraglenoid tubercle while the short head arises from the coracoid process, the two bellies merging as they descend. Distally, the biceps tendon courses inferiorly toward the radial tuberosity, with the bicipital aponeurosis fanning medially across the cubital fossa and blending into the antebrachial fascia, while the brachialis remains visible deep to biceps and approaches the ulnar tuberosity and coronoid process. Orientation is clean. Medial relationships read first. This medial perspective matters because it clarifies what biceps does and what it does not do at the elbow: brachialis is the primary flexor regardless of forearm rotation, while biceps contributes most when the forearm is supinated and also drives powerful supination via its insertion on the radius. The broad bicipital aponeurosis, often glossed over in lateral views, frames the neurovascular corridor of the cubital fossa and is a key landmark during antecubital venipuncture or surgical exposure for distal biceps tendon repair. Distal biceps ruptures classically retract proximally and leave the aponeurosis variably intact, a detail that changes the clinical exam and operative plan.
