Seen from the posterior forearm, the extensor digiti minimi lies in the superficial extensor compartment, positioned medial to the extensor digitorum and lateral to the extensor carpi ulnaris as it courses distally toward the wrist. Its muscle belly tapers into a long tendon that passes beneath the extensor retinaculum on the dorsum of the wrist, then tracks toward the ulnar side of the hand to reach the fifth digit. The tendon aligns dorsal to the metacarpals and phalanges, where it typically joins the extensor expansion of the little finger. For teaching and procedural planning, a posterior view makes the extensor digiti minimi easy to separate conceptually from the shared tendons of extensor digitorum, which matters when parsing independent extension of the small finger during exam or during tendon transfer. Rupture or attritional wear at the level of the distal radioulnar joint and dorsal wrist, including in rheumatoid arthritis, can selectively affect ulnar-sided extensor tendons and produce loss of active extension at the fifth metacarpophalangeal joint. Palpation and dynamic assessment over the dorsoulnar wrist, just ulnar to the extensor digitorum tendons, often hinges on understanding this tendon’s path through the extensor compartments.
