Description
Running along the posteroulnar forearm, the extensor digiti minimi (EDM) is singled out beneath the skin as a narrow fusiform muscle arising from the common extensor origin at the lateral epicondyle and coursing distally alongside the extensor digitorum. Its tendon travels posterior to the distal ulna, passes deep to the extensor retinaculum within the fifth dorsal compartment, and continues onto the dorsum of the hand toward the little finger. Radial to it, the extensor digitorum tendons form a broader central band over the dorsal wrist, while the ulnar head and styloid sit just deep and distal as palpable landmarks. Clean topography. Superficial soft tissue planes remain evident. Isolated visualization of the EDM matters because its function is often taught as part of the “mobile wad” and posterior compartment patterning, yet clinically it behaves like a discrete tendon with its own compartment, synovial sheath, and common sites of irritation at the dorsal wrist. Overuse tenosynovitis in the fifth compartment can mimic extensor carpi ulnaris pathology, and an accessory slip of EDM may complicate dorsal wrist surgery or ultrasound interpretation when evaluating snapping or focal tendinopathy. Hand surgeons also exploit the EDM tendon for tendon transfer or reconstruction when restoring extension balance of the ulnar digits after extensor tendon injury.
