Description
Running longitudinally on either side of the spinous processes, the multifidus (musculus multifidus) appears as a deep transversospinales muscle spanning the posterior vertebral column from the sacrum and posterior iliac crest toward the cervical region. Muscle bellies and short fascicles are layered medial to the longissimus and iliocostalis components of the erector spinae, occupying the groove between the spinous and transverse processes. Inferiorly, the fibers blend with the posterior sacroiliac region near the sacrum and posterior superior iliac spine, while superiorly they track segment by segment along the laminae. A deep stabilizer, not a superficial contour. Posterior views like this matter when you need to teach segmental lumbar stability rather than gross trunk extension. Multifidus atrophy and fatty infiltration are well described after acute and chronic low back pain episodes and can persist after resolution of symptoms, which is why rehabilitation programs often target multifidus activation instead of only strengthening the superficial erector spinae. The illustration also supports procedural anatomy: medial branch blocks and radiofrequency ablation for zygapophysial (facet) pain traverse tissue planes that include multifidus, and postoperative denervation can contribute to visible multifidus wasting on follow-up imaging.
