Description
Running bilaterally along the posterior vertebral column, the multifidus forms a segmented series of short muscle slips spanning adjacent vertebrae from the sacrum and posterior iliac spine region up through the lumbar, thoracic, and cervical spine. Individual fascicles lie deep and medial to the erector spinae mass, crossing from transverse processes and articular processes to spinous processes, with the thickest, most clearly partitioned bundles typically seen in the lumbar region. Surrounding skeletal landmarks include the occiput superiorly, the thoracic cage laterally, and the posterior pelvis and sacrum inferiorly, providing orientation for the muscle’s longitudinal course. A posterior perspective like this is the teaching view that makes the multifidus understandable: short, overlapping stabilizers rather than a single strap muscle. That distinction matters in low back pain and post-lumbar surgery care, where multifidus atrophy and fatty infiltration correlate with segmental instability and recurrent symptoms, and where paraspinal muscle-splitting approaches aim to limit denervation and ischemic injury. Palpation and motor control retraining often target the lumbar multifidus specifically, just lateral to the spinous processes at L4 to L5. Small muscle, big consequences. Use this illustration in spine anatomy and kinesiology modules to contrast deep transversospinal muscles with the more superficial erector spinae, and in orthopedic or neurosurgical texts discussing posterior lumbar approaches (midline vs paramedian) and postoperative paraspinal changes on MRI.
