Viewed laterally in the cervical region, the multifidus appears as a series of thick, obliquely oriented fascicles occupying the paraspinal groove immediately lateral to the spinous processes of the cervical vertebrae. Deep to more superficial posterior neck muscles, its segmental bundles run superomedially from mammillary or articular regions and transverse processes toward the spinous processes of vertebrae superior to their origin, placing the muscle medial to the longissimus cervicis and lateral to the midline nuchal ligament and spinous apices. Short intersegmental slips sit deepest, with longer slips spanning multiple vertebral levels more superficially within the same muscle mass. Fiber direction is clear. For teaching spinal biomechanics, this lateral perspective matters because the cervical multifidus is one of the most clinically discussed deep stabilizers of the neck, and it is frequently implicated in persistent neck pain after whiplash-associated disorders where atrophy and fatty infiltration can be demonstrated on MRI. Surgeons and pain specialists also reference this anatomy during posterior cervical approaches, since the multifidus must be split or elevated from the spinous processes and laminae, and preserving its attachments is linked with reduced postoperative axial neck pain.
