Description
Posteriorly, the thoracoabdominal course of the esophagus is traced along the midline as it descends through the posterior mediastinum to the esophageal hiatus of the diaphragm, then continues into the abdominal cavity to join the stomach at the cardia. The stomach lies in the left upper quadrant, with the fundus positioned superior and left of the gastroesophageal junction and the body extending inferomedially toward the antrum and pyloric region. Along this back-facing orientation, the organ’s posterior surface (gastric bed) is implied, where the stomach sits anterior to retroperitoneal structures and the posterior abdominal wall. Clear topography. Teaching the stomach from behind forces attention onto relationships that matter clinically but are often lost in an anterior schematic, namely the proximity of the posterior gastric wall to the pancreas, splenic vessels, left kidney and adrenal, and the diaphragmatic crura at the hiatus. This is the anatomy behind posterior penetrating gastric ulcers, which can erode into the pancreas and present with back pain, or into the splenic artery with brisk upper gastrointestinal hemorrhage. The posterior view also reinforces why hiatal hernia and gastroesophageal reflux disease are not just mucosal problems, because the angle of His and the diaphragmatic hiatus frame the gastroesophageal junction in three dimensions.
