Description
Seen in a layered microscopic profile, the scalp overlies the calvarial bone, followed immediately by the dura mater lining the inner table of the skull. Deep to the dura, the arachnoid mater spans the intracranial cavity as a translucent membrane, with fine arachnoid trabeculae coursing inferiorly to the pia mater. A CSF-filled subarachnoid space separates arachnoid from pia, while the pia closely invests the cortical surface and follows sulci, sitting directly superficial to the superficial cerebral parenchyma. Small arteries and veins track within the scalp, along the meningeal coverings, and on the cortical surface. That relationship between dura, arachnoid, and pia is where common intracranial hemorrhage patterns make anatomical sense. Bridging veins traverse from the cortical venous surface across the subarachnoid space toward dural venous sinuses, and rupture in acceleration injuries produces subdural hematoma, a bleed that collects deep to the dura rather than within the CSF space. Blood confined to the subarachnoid space instead mixes with CSF and tracks through cisterns and sulci, the classic distribution after aneurysmal subarachnoid hemorrhage and a key reason why lumbar puncture can reveal xanthochromia when early CT is negative.
