Description
Viewed from the anterior aspect with a basal orientation, the arachnoid mater drapes over the inferior surfaces of the cerebral hemispheres, bridging across sulci rather than dipping into them. The longitudinal fissure remains a midline cleft between the hemispheres, while the translucent arachnoid spans over the interhemispheric region and toward the parasellar area where cranial nerves converge. Anteriorly and slightly inferiorly, the optic nerves (CN II) course toward the optic chiasm, and more posterior and central the brainstem emerges, with the cerebellum positioned inferior and posterior to the cerebral hemispheres. Fine superficial vascular impressions suggest subarachnoid space pathways over the cortical surface. A basal anterior view matters because it aligns the meningeal layers with the skull base corridors that clinicians navigate and image: the arachnoid sits deep to dura mater and superficial to pia mater, and it defines the subarachnoid space where cerebrospinal fluid circulates and where arteries and cranial nerves travel. Blood in this space after rupture of a saccular aneurysm often accumulates around the basal cisterns, tracking along the optic chiasm and brainstem rather than remaining confined to a single sulcus. Small detail, big consequence. The same anatomy explains why leptomeningeal infection or carcinomatosis can present with multiple cranial neuropathies, reflecting disease spread along arachnoid-lined cisterns and nerve root sleeves.
