Description
Centered in the mediastinum, the heart sits posterior to the sternum with the ascending aorta arising superiorly and curving leftward as the aortic arch before continuing as the descending thoracic aorta. Branches from the arch, the brachiocephalic trunk, left common carotid artery, and left subclavian artery, course superiorly into the neck and laterally into the upper limbs, where they continue as the axillary and brachial arteries. Venous return parallels these routes: the internal jugular and subclavian veins unite to form the brachiocephalic veins, which converge to the superior vena cava on the right side of the upper mediastinum. Pulmonary arteries and veins connect the heart to the lungs at the hila, framing the great vessels in a clear anterior, radiographic style. Spatially, this arrangement matters because many high-stakes interventions in the chest assume you can predict where these vessels lie relative to the manubrium, clavicles, and right atrium, even when the body surface is all you have. Central venous catheterization, pacemaker lead placement, and management of thoracic aortic aneurysm all depend on recognizing the course and confluence of the brachiocephalic veins and the sweep of the aortic arch. A clean anterior overview also helps teach why mediastinal masses, aortic dissection, or an enlarged left atrium can shift expected silhouettes on chest x-ray.
