Rendered from a posterior perspective, the upper cervical region is shown beneath a translucent skin layer, with the trapezius forming the most superficial muscular sheet and the rhomboid minor, rhomboid major, and levator scapulae emerging deep to its medial and superior fibers. Superiorly, the suboccipital group (rectus capitis posterior major and minor, obliquus capitis superior, and obliquus capitis inferior) occupies the space between the occiput and the posterior arch of atlas, framing the upper cervical spine. Inferior and medial to these, deeper extensors such as splenius capitis and semispinalis capitis align longitudinally toward the spinous processes of C2 to C7, with the nuchal ligament and palpable spinous tips establishing midline orientation. Clear landmarks. This posterior layering matters when teaching cervical biomechanics and when planning posterior approaches to the craniovertebral junction, where the relationship of the suboccipital triangle to the vertebral artery and the suboccipital nerve informs safe dissection planes. It also maps directly to common pain generators, including greater occipital nerve irritation near the superior nuchal line and myofascial trigger points in levator scapulae and upper trapezius that refer pain to the occiput and temporal region. The juxtaposition of muscle fibers and cervical spinous processes also supports interpretation of MRI and ultrasound-guided injections by giving anatomic context to depth and laterality.
