Description
Posterior skin reflection reveals the layered extensor compartment of the male neck and upper thorax, with trapezius forming a broad superficial sheet from the superior nuchal line and external occipital protuberance to the spinous processes and scapular spine. Deep to it, splenius capitis runs superolaterally from the nuchal ligament and upper thoracic spinous processes to the mastoid process and lateral superior nuchal line, while semispinalis capitis ascends more medially toward the occipital bone between the nuchal lines. Along the midline, the nuchal ligament anchors into the cervical spinous processes, and the cervical to upper thoracic spinous processes provide palpable bony reference points inferiorly; the suboccipital muscles sit most superiorly, tucked beneath the occiput around the posterior arch of C1 and the spinous process of C2. Posterior deep neck anatomy matters when teaching the mechanics of extension and ipsilateral rotation, but it becomes clinical when pain localizes to the upper cervical region, where sustained contraction of suboccipital and semispinalis capitis fibers often contributes to cervicogenic headache patterns. This view also supports procedural orientation: midline approaches to the cervical spine and posterior elements rely on recognizing the nuchal ligament as a relatively avascular plane, while lateral dissection risks the greater occipital nerve as it courses through semispinalis capitis near the superior nuchal line. Landmarks are everything.
