Description
Positioned deep to splenius capitis and trapezius, the semispinalis cervicis is presented bilaterally along the posterior cervical spine, flanking the midline spinous processes from the upper thoracic region toward the mid cervical vertebrae. Muscle fascicles course superomedially from transverse processes of T1 to T6 to insert on the spinous processes of C2 to C5, so the fibers converge as they ascend. Surrounding bony landmarks include the occiput and posterior skull base superiorly, the cervical and upper thoracic vertebral arches centrally, and the scapulae laterally over the posterior rib cage. Paired, columnar anatomy. This posterior exposure matters when teaching the transversospinales layer and when planning posterior cervical approaches, because semispinalis cervicis sits in the dissection plane once the more superficial epaxial muscles are reflected. The C2 spinous process is a practical landmark: semispinalis cervicis commonly inserts there and is frequently encountered during posterior fixation, decompression, or muscle-sparing approaches aimed at preserving cervical extension strength and limiting postoperative axial neck pain. The fiber direction also helps distinguish it from semispinalis capitis, whose bulk continues to the occipital bone and can obscure the deeper cervicis slips unless the dissection stays close to the spinous processes.
