Description
Anterior cervical anatomy is presented with the skull, mandible, and upper thorax framing a focused view of the posterior scalene muscle in a male. Deep in the lateral neck, the posterior scalene lies posterior to the anterior and middle scalenes, descending from the cervical transverse processes to its inferior attachment on the second rib, just posterior to the first rib and the scalene tubercle region. Medially, the muscle relates to the cervical vertebral column and intervertebral discs, while laterally it borders the supraclavicular fossa above the clavicle and the upper ribs. Because the posterior scalene is smaller and more variable than the anterior and middle scalenes, an anterior perspective that still tracks its fiber direction helps clarify where it sits relative to the thoracic inlet. This is a key landmark region for thoracic outlet syndrome teaching, where scalene hypertrophy, fibrous bands, or altered first rib mechanics can contribute to brachial plexus or subclavian vessel compression, and where palpation and ultrasound-guided injections are planned with attention to nearby neurovascular structures. The rib attachments matter. They influence elevation of ribs 1 and 2 during forced inspiration and can contribute to cervicothoracic junction pain patterns.
