Dura Mater

Description

Laterally oriented anatomy of the left cranial cavity emphasizes the dura mater as it lines the endocranial surface and invests the brain, with the arachnoid mater and pia mater implied as the deeper meningeal layers on the cerebral surface. Superiorly and posteriorly, the dura follows the convexity of the cerebral hemispheres beneath the calvaria, while inferiorly it continues toward the foramen magnum to become continuous with spinal dura around the cervical spinal cord. Yellow nerve elements track from the brainstem and along the cervical region as cranial nerves and spinal nerves, and red and blue vessels course in parallel, consistent with arterial supply and venous drainage adjacent to the meninges. Bony landmarks of the skull and upper vertebral column provide orientation in relation to the central nervous system. Understanding dura matters because it is the operative plane for many neurosurgical approaches and the key barrier involved in extra-axial hemorrhage. Epidural hematoma classically forms between the skull and periosteal layer of dura, often after temporal bone trauma with middle meningeal artery injury, while subdural hematoma accumulates deep to dura at the dural border cell layer after tearing of bridging veins. Headache patterns and cranial neuropathies also make more sense when you recall dural innervation (trigeminal nerve anteriorly, upper cervical nerves posteriorly) and the close relationship of cranial nerves and dural folds at the skull base.
