Description
Running longitudinally along the posterior thoracolumbar trunk, the erector spinae mass is organized into iliocostalis laterally, longissimus in the intermediate column, and spinalis closest to the spinous processes. Inferiorly, fibers arise broadly from the posterior iliac crest, dorsal sacrum, and thoracolumbar fascia, then ascend to the ribs and cervical region. Medially, the spinalis tracks adjacent to the vertebral midline, while iliocostalis follows the rib angles and the lateral border of the lumbar region. Functionally, this layered arrangement explains regional differences in extension, lateral flexion, and segmental stabilization, from the lumbar spine up to the lower cervical levels. Clinically, the thoracolumbar fascia and the deep erector spinae compartment matter in posterior spinal approaches and in interpreting paraspinal muscle atrophy on MRI in chronic low back pain. Palpation and needling targets also change across columns, with longissimus often approached just lateral to the spinous processes while avoiding the medial branches of the dorsal rami and adjacent facet joints. A common pain generator.
