Running longitudinally along the posterolateral thoracic wall, the iliocostalis thoracis appears as slender muscle slips positioned lateral to the thoracic spinous processes and medial to the rib angles. Adjacent ribs articulate with the thoracic vertebrae at the costovertebral and costotransverse joints, placing the iliocostalis thoracis immediately superficial to the posterior rib cage as it tracks superiorly toward the upper ribs. Laterally, the scapulae sit over the rib cage, with the medial borders lying superficial to the posterior ribs and separated from the paraspinal compartment by intervening musculature. The erector spinae mass is the organizing landmark. Functionally, isolating the iliocostalis thoracis helps clarify rib mechanics during forced respiration and trunk extension, because its fibers couple posterior rib elevation with ipsilateral lateral flexion of the thoracic spine. Clinically, this region is a frequent source of paraspinal myofascial pain and rib-related referral patterns, and it is also the tissue plane encountered during posterior thoracic approaches where retraction near the rib angles and costotransverse junctions can irritate the dorsal rami.
