Description
Running along the lateral compartment of the male leg, the fibularis (peroneus) longus muscle belly sits superficial and slightly posterior to the fibula, with its tendon descending distally toward the lateral malleolus. Medially, the tibia defines the shin and forms the medial malleolus at the ankle, while the fibula remains the lateral strut from the fibular head near the knee down to the ankle mortise. At the level of the ankle, the fibularis longus tendon courses posterior to the lateral malleolus, then turns inferomedially across the plantar foot toward its insertion on the base of the first metatarsal and medial cuneiform. Bones of the hindfoot and forefoot (talus, calcaneus, tarsals, metatarsals, phalanges) provide clear landmarks for the tendon’s change in direction. Clinically, this tendon path is where things go wrong. Repetitive inversion injury or a torn superior fibular retinaculum can allow peroneal tendon subluxation around the lateral malleolus, and longitudinal splits of the fibularis longus often coexist with peroneus brevis pathology in chronic lateral ankle pain. Proximally, the relationship to the fibular head also matters, since the common fibular (peroneal) nerve wraps around the neck of the fibula and is vulnerable during trauma or surgical exposure.
