Description
Presented in an anterior full-body orientation, the lateral compartment of the leg is brought forward, centering on the fibularis (peroneus) longus muscle belly along the proximal two thirds of the fibula. The muscle lies lateral to the tibia and anterior to the posterior compartment, with its distal fibers narrowing into the fibularis longus tendon as it approaches the lateral malleolus of the fibula. From this angle, the tendon’s course can be appreciated as it passes posterior to the lateral malleolus before turning plantarward toward the cuboid groove and continuing obliquely across the sole to the base of the first metatarsal and medial cuneiform. Superficial landmarks matter. Clinically, this view is a workhorse for teaching lateral ankle anatomy because fibularis longus shares a common synovial sheath with fibularis brevis behind the lateral malleolus, a site prone to tenosynovitis and tendon subluxation after inversion injuries. Orthopedic and sports medicine readers also track the tendon’s plantar turn when discussing pain under the cuboid (so-called cuboid syndrome) and altered first-ray plantarflexion mechanics, which can contribute to lateral column overload. Palpation and resisted eversion testing follow the same surface line shown here, from the fibular head region down to the retromalleolar groove. Use this illustration in gross anatomy and kinesiology courses to anchor compartmental organization of the leg, or in a sports medicine handout explaining peroneal tendon disorders and lateral ankle stabilization.
