Description
Semi-transparent integument and superficial musculature frame the right upper quadrant, where the gallbladder sits on the visceral (inferior) surface of the liver, just inferior to the right costal margin. The fundus projects anteroinferiorly, the body lies more posterior and superior in its fossa, and the neck tapers medially toward the porta hepatis to continue as the cystic duct. In continuity, the biliary tree is indicated in green, with the common hepatic duct joining the cystic duct to form the common bile duct as it descends posterolateral to the first part of the duodenum toward the pancreatic head. Adjacent stomach and proximal small bowel provide spatial context. Clinical relevance centers on where pain and obstruction occur. Gallstones commonly lodge at the neck or cystic duct, producing biliary colic and, when persistent, acute cholecystitis with a tender gallbladder fundus near the right midclavicular line (Murphy sign). The highlighted ductal course also supports teaching of obstructive jaundice and pancreatitis, since a stone at the distal common bile duct or ampulla of Vater can dilate the extrahepatic ducts and secondarily inflame the pancreas; this is the same anatomy navigated during ERCP and during the critical view of safety in laparoscopic cholecystectomy to avoid common bile duct injury.
