Description
Oriented from a posterior aspect, the second part of the duodenum descends on the right side of the abdomen with the head of the pancreas nestled medially in its C loop. The common hepatic duct and cystic duct converge to form the common bile duct, which courses inferiorly and posterior to the first part of the duodenum before approaching the posteromedial wall of the descending duodenum. Within the hepatoduodenal ligament superiorly, the biliary ducts arborize into right and left hepatic ducts at the porta hepatis, while the gallbladder sits inferior to the liver and connects via the cystic duct to the extrahepatic biliary tree. At the duodenal wall, the hepatopancreatic ampulla (ampulla of Vater) opens at the major duodenal papilla, the key junction where bile and pancreatic exocrine secretions enter the gut lumen. A posterior view clarifies relationships that drive symptoms and guide operative planning, because the distal common bile duct and ampulla lie deep, near the pancreatic head, and pathology here often presents with obstructive jaundice and cholangitis. Gallstones passing into the common bile duct can impact at the ampulla, provoking gallstone pancreatitis, while malignant obstruction at the pancreatic head classically compresses the intrapancreatic bile duct. Endoscopic retrograde cholangiopancreatography (ERCP) targets the major papilla for cannulation, sphincterotomy, and stent placement. Small distances matter. Use this rendering to teach the extrahepatic biliary anatomy in gross anatomy lab, GI blocks, and hepatobiliary surgery modules, or to illustrate ERCP orientation, choledocholithiasis, and pancreatic head carcinoma in textbooks, patient education, and clinical presentations.
