Description
Dominating the posterior hip, the gluteus maximus forms the superficial muscular mass of the male buttock, spanning from the posterior ilium, dorsal sacrum, and coccyx to its distal attachments on the iliotibial tract and the gluteal tuberosity of the femur. Superiorly, the lower lumbar vertebrae transition into the sacrum at the lumbosacral junction, with the sacroiliac region positioned deep and medial to the muscle belly. Laterally, the greater trochanter sits inferior to the iliac crest, giving a bony reference for the proximal femur as the gluteal fibers run inferolaterally toward their insertions. Skin has been removed. This posterior superficial dissection view matters because it places gluteus maximus in direct relationship to the sacrum, iliac crest, and proximal femur, the landmarks clinicians palpate when assessing gait, hip extension strength, and pelvic stability. Hip extensor weakness from inferior gluteal nerve injury, or pain after posterior-approach hip arthroplasty, makes more sense when you can track how the muscle’s broad origin converges toward the iliotibial tract and femur while covering the deeper short external rotators and the course of the sciatic nerve. The upper outer quadrant injection site also becomes a practical teaching point when the contours of the buttock are tied back to the underlying pelvis. Use this illustration in gross anatomy and kinesiology courses to explain hip extension, external rotation coupling, and the load transfer between trunk and lower limb at the sacroiliac region during rising from a chair or stair climbing. It also fits well in orthopedic and sports medicine texts discussing gluteal strain, tendinous insertions at the femur, and postoperative anatomy after posterior hip exposure.
