Description
Across the lateral surface of the ilium, the gluteus minimus is rendered as a thin, fan-shaped muscle arising between the anterior and inferior gluteal lines and converging inferiorly toward the anterior aspect of the greater trochanter of the femur. Deep to where the gluteus medius would lie, its fibers span from the iliac wing down to the proximal femur, sitting lateral to the acetabulum and hip joint capsule. Posteriorly the sacrum and coccyx anchor the pelvic ring, while superiorly the lower lumbar vertebrae and inferior rib margins establish the lumbopelvic context for hip abductor mechanics. Clinically, this deep abductor matters because loss of gluteus minimus and medius function produces pelvic drop on the contralateral side during single-leg stance, the basis of a Trendelenburg gait. The anterior insertion on the greater trochanter places the muscle in the operative field for lateral and anterolateral hip approaches, and it is also one of the tendons implicated in greater trochanteric pain syndrome and partial tears that can mimic hip joint pathology. Small, deep, and easy to overlook. This view keeps its relationship to the ilium and proximal femur unambiguous. Use it in gross anatomy and musculoskeletal medicine teaching when you need to separate the deep hip abductors from the more superficial gluteal mass, or in orthopedic and sports medicine publications discussing abductor tendon pathology, gait deviation, and postoperative abductor weakness after total hip arthroplasty. It also supports patient-facing education on why lateral hip pain may originate from abductor tendinopathy rather than the joint itself.
